Amulet of the Planes

Wondrous Item, Very Rare (requires attunement)

Aura: Strong Conjuration 

While wearing this amulet, you can use an action to transport yourself instantly to a different plane of existence. If you know the sigil sequence of a teleportation circle on another plane of existence, then you can transport yourself there. If not, you appear in a random location on a random plane, determined by rolling a d100 and consulting the table below. Once the amulet has been used, it cannot be used again until the following dawn.

One more item from the 2e Encyclopedia Magica converted to 5e. This one is very similar to the existing 5e version of Amulet of the Planes, only a bit more chaotic and random- pretty much on brand for older edition items.
